Here is a copy (see above link) in Adobe Acrobat of the FHE Recipe Booklet I created for a Primary Activity Day.  NOTE: I have changed the bottom to read, "Made with Love from Our Family to Yours"

Instructions after printing all pages from your computer:

1. Photocopy first page onto colored cardstock.

2. Photocopy the remaining pages front to back, on white paper, in the following order. NOTE: 16/Index is the page with 16 on the left side and the Index on the right side:

  • Page 2/15 on front side, Page 16/Index on the back side.
  • Page 4/13 on front side, Page 14/3 on its back side.
  • Page 6/11 on front side, Page 12/5 on its back side.
  • Page 8/9 on front side, Page 10/7 on its back side.
When done, you will have a cover and four double-sided copies.

3. Assemble booklet as follows:
  • Put copied side with pages 2/15 showing down first (the 16/Index side will be facing table).
  • Follow with 4/13 showing placed on top of 2/15.
  • Next put 6/11 on top.
  • Last, put 8/9 on top of that.

When folded in half, you will have a booklet from Index through page 16. Add cardstock cover to the outside of the booklet and staple. I have one of those long-handled staplers that is perfect for booklets.
